Registration Forms

Use the forms below to enroll in or drop Cal State East Bay Continuing Education courses, or to apply for one of our many certificate programs.

Please Note: These forms cannot be used for Open University or regular University courses or by International Students.

Courses

Registration Form (PDF, 71k)
Complete and submit this form by fax, mail or in person to register for courses listed in our course catalog.

Alternately, you may also use our convenient registration portal to select your courses and then use the "Print Registration" function on the shopping cart page to print out a registration form already completed with your course selection(s).

Course Transfer (PDF, 31k)
To request a transfer from one course to another, complete and return this form.

Withdrawal/ Refund (PDF, 89k)
Note: This form is only for use with Continuing Education courses. To withdraw from a regular University course, visit the Enrollment Services forms page and download your form there. Withdrawal from a course requires an official, signed statement, either on this form or in a separate letter. Refunds issued are based upon the time a withdrawal statement is submitted to our offices. Please read our withdrawal/refund policy for more information.

Certificate Programs

Program Application (PDF, 47k)
To enroll in a certificate program, you must complete and return this form along with a $40.00 application fee. Most programs require you to submit this form no later than your second quarter of enrollment in the program.

Petition for Certification (PDF, 33k)
To obtain your official certificate, you must complete all requirements of your program and then complete and submit this form. Upon verification of requirement completion, your certificate will be mailed to you.
